Hope Rising executive director and Lake County Wellville coordinator Shelly Trumbo was among the leaders who attended last week’s “Putting Care at the Center 2018,” an annual conference on multi-sector collaboration for complex care organized by Camden Coalition of Healthcare Providers. At the event, which was oriented around transformation systems, Shelley presented Project Restoration, an initiative that provides intensive case management services for countywide “super-utilizers” – residents who are frequent users of two or more county services. There is so much in our hearts and minds following these tumultuous past few months and intense past few days. This is a moment to consider the causes of the consequences that are now on full display. It’s time to call out what led to such health disparities and what will it take to improve outcomes for all.
