Hope Rising executive director and Lake County Wellville coordinator Shelly Trumbo was among the leaders who attended last week’s “Putting Care at the Center 2018,” an annual conference on multi-sector collaboration for complex care organized by Camden Coalition of Healthcare Providers. At the event, which was oriented around transformation systems, Shelley presented Project Restoration, an initiative that provides intensive case management services for countywide “super-utilizers” – residents who are frequent users of two or more county services.
